General Information
C-section scars oftentimes heal indented because the tissue is not closed in the appropriate layers. Directly under the skin is a layer of fat which is divided into two separate sections by a layer of relatively dense tissue called Scarpa’s fascia. This tissue is usually not closed in a c-section, so the fatty tissue separates, causing the skin to heal directly to the muscle. Since there isn’t fat under the scar, the scar indents and the fatty tissue then hangs over the scar, causing a “pooch.”
There are a variety of different techniques used by Dr. Elswick for scar revision. The general procedure is to cut out the old scar and then close the tissues in a layered fashion. A plastic surgeon is highly trained in closing incisions to minimize the appearance of the scar, but the scar will always be present. C-section scar revision can be combined with other procedures such as liposuction, Renuvion skin tightening, or even a tummy tuck to enhance the outcome.
Planning your Surgery
During your initial consultation Dr. Elswick will discuss your desired results and evaluate the structure of your body. With this information she will be able to develop a plan that fits your goals. This procedure should be completed at least six months post-partum once you are at a stable and comfortable goal. It is also recommended to postpone surgery if you are planning any future pregnancies. If you have a significant amount of excess skin or fat you may be a better candidate for a tummy tuck and/or liposuction.
Preparing for your surgery
Our qualified staff will give you specific instructions on how to prepare for surgery, including guidelines on diet, medications, activity level, and nicotine use. Detailed instructions on activity before and after surgery will be discussed.
If you smoke or use other nicotine products, plan to quit at least 4 weeks before the scheduled surgery. Smoking increases the risk of complications after surgery and poor wound healing will occur. Avoid sun exposure for at least 8 weeks before and after surgery as this may worsen scar appearance.
What is the procedure like?
A c-section scar revision can be under local anesthetic (numbing medicine) with oral or IV sedation if desired. The surgery takes about an hour and is performed in our outpatient surgery center.
What can I expect after surgery?
Patients typically do very well after c-section scar revisions. Most patients have minimal discomfort that is treated with Tylenol and NSAIDs. Generally you can return to normal exercise and activities after 2 weeks. You can return to work the next day unless you work a job that requires heavy lifting. Minimal swelling is anticipated will improve significantly within the first 4 weeks after surgery. The scar will continue to mature and fade for 1-2 years after your procedure.
Risks associated with c-section scar revision.
Every surgical procedure involves a certain amount of risk, and it is important that you understand these risks and the possible complications associated with them. In addition, every procedure has limitations. An individual’s choice to undergo a surgical procedure is based on the comparison of the risk to potential benefit. Although the majority of patients do not experience complications, you should discuss them with Dr. Elswick to make sure you understand all possible consequences of c-section scar revision.
