On the outside someone may appear to be calm and relaxed however, an interior view of their mouth tells a very different story.  One of the exciting things about medicine is often we discover that a particular drug can offer benefits to several other aliments then its intended purpose. I suffer from unconscious clenching and grinding of my teeth both during the day and as I sleep. While attending a training workshop I heard Botulinum toxins found in products such as Botox® and Dysport® can be used to relax and relieve the masseter muscle.

After further researching bruxism (the formal name for this condition) I discovered I was not alone and this is a common and painful problem that can lead to severe dental issues. Chronic teeth clenching/grinding can cause headache, earaches, facial pain, and even migraines. Bruxism sufferers who grind and clench their teeth while sleeping frequently wake up with a sore jaw.  The good news is treatments of Botox/Dysport injected into the masseter muscle can provide tremendous relief from jaw soreness, headaches, and other unpleasant problems.  The administration technique is the same which is used for wrinkle reduction. Usually three small injections are administered into the muscle on each side. This allows the muscle to relax and ease the tension in the jaw.  It may last 3 to 5 months.

It can also be used for Aesthetic corrections.  Individuals suffering from hypertrophy of the masseter muscle, can have the appearance of a severe square jaw, giving a more masculine appearance to the face.  Injecting Botulinum into this area can soften the jawline and help to reverse this.

Does the loss of volume in your face have you feeling down or do you keep hearing people tell you that you have a frown?  I see patients all the time complaining that they feel like their faces are falling or that they are getting “jowly,” or look like they are constantly frowning. These complaints are usually due to us subtly or not so subtly noticing signs of aging on our face.   These signs of aging usually start with changes in the skin, fat and muscle layers as well as changes to the bone structure itself.

The perceived volume loss occurs due to fat redistribution, and the bones become thinner and flatter in places.   As we age, this fat/volume loss in the face causes the face to appear “saggy.” Many patients come in wanting to fill in the nasolabial folds and marionette lines, when often times, restoring volume to the cheek area would be much more beneficial.  Injecting filler into the lower face may not always be a good idea, depending on the patient, because it may possibly accentuate the heaviness a patient is feeling. As we age our faces lose their shape. This causes your heart, oval, or round shape to become more square and bottom heavy.  This facial shape of aging can many times be reversed to some degree by restoring volume to the mid-face. This in turn will impact the appearance of the lower face.

Derma filler such as Voluma ® or Restylane Lyft ® to the mid-face will also likely help lift the nasolabial folds and the marionette lines in the lower face as well as at times minimize or improve the appearance of jowls. Do you dread summer because you can’t raise your arms in the air without showing
wet perspiration stains? Have you tried every deodorant on the store shelves and
nothing seems to control your perspiring? If you answered yes, then I have a
suggestion for you and it’

s Botox®. I’m sure many of you are thinking Botox is for
the wrinkles on my face not my sweaty underarms. It is used for both areas. Studies
have shown that Botox treats overactive sweat glands in your armpits (medically
known as hyperhidrosis) by blocking the uptake of acetylcholine, one of the
neurotransmitters that stimulates sweat.


When Botox is used in the face it freezes the nearby muscles which smoothes
wrinkles and prevents expression lines that could cause even more wrinkles to
form. It is a similar mechanism that is at work when Botox is used to stop
perspiration. Instead of working on nerves that stimulate the muscles, it blocks the
nerves controlling sweat glands essentially “paralyzing” them and significantly
reducing the amount you sweat under your arms. Injecting Botox into this area will
not affect other areas of the body. Your body can still sweat through other areas to
regulate its internal temperature.

If you like the sound of this your next question is likely, what is this process like?
The entire process is usually scheduled as a 30 minute appointment. We will
perform a simple starch iodine test to see where the sweat glands are to better
target the most active glands. Once the area is identified it is cleaned off and the
Botox is injected in small doses. The pain level is tolerable and similar to Botox
injections into facial areas. Most common side effects are bruising and possibly a
little tenderness.

In a clinical study by Ada Regina Trindade de Almeida, MDa,*, Suelen Montagner, MDb, the average
duration of response in patients receiving a Botox treatment for hyperhidrosis was
6 months.

Sculptra Butt Filler

Wanted… Fuller buttocks and filled in dimples!

As an Aesthetic RN, ISSA Certified and Competitive IFBB Bikini Pro, I receive messages asking what can be done for a flat buttock, dimpling or single cellulite dimples. First I recommend muscle building exercises targeted for this area such as squats. However, if squats in the gym aren’t giving the lift you need or genetics are against you, say no more. A non-surgical solution is Sculptra injections.

What is it?

Sculptra® Aesthetic works subtly and gradually over time for a more youthful-looking appearance. Sculptra® is a poly-L-lactic-acid-based injectable dermal filler. It is comprised of polylactic acid, which has been used in the medical field for over 20 years in surgical products, which includes dissolvable stitches. Polylactic acid is a synthetic substance made to duplicate a similar product that the body’s muscles make during exercise. When injected it simulates an injury that will stimulate collagen production to the area to heal the injection site.  

How it works 

 

Sculptra’s microspheres generate a mild inflammatory reaction which stimulates your collagen.  Collagen will gradually increase, creating the volume-enhancing results & a lifted appearance which is visible from the treatment in about four to six weeks. Sculptra is FDA approved for use in shallow/deep wrinkles, nasolabial folds and contour deficiencies. Many aesthetic products are introduced to the public with FDA approval for use in a certain body area. Doctors however, will discover that the products can often work in other areas of the body as well and produce desired results.  This is referred to as “Off Label” use. One of these off label uses is injecting Sculptra into the buttock area or into dimples to increase volume by stimulating collagen production.

I typically like to start off by injecting about 4 vials each side, and can go up to 10 vials for complete treatment, with a return visit at 4-6 weeks. I have patients that are athletes who stay lean but would like more fullness or contouring. The best candidates are ones that have low body fat or no fat to harvest and don’t want the downtime of a surgical implant or fat grafting. It is important to note that depending on the fullness desired the total for the procedure can be the higher price range but is an alternative to surgery. Sculptra is often used off label as well to smooth cellulite indentations or dimples in the skin that won’t go away for a smoother appearance.

What are the most common side effects after Sculptra®? Discomfort at injection site, burning, redness, swelling, nodules and bruising. The luxury of this treatment is no recovery period vs surgery interfering with your daily life.

 

How long does it last?

Sculptra is a product that generates your own collagen. It’s different than HA (hyaluronic acids) which only only last 9-18 months. Results may vary per patient with Sculptra fillers, with a range of lasting 2-5 yrs.
