ResurFX -Skin Resurfacing wit NO downtime!

 

The ResurFX Laser is my favorite! It is a non-ablative laser so there is little to no downtime with great results.  The ResurFX uses heat to stimulate the growth of collagen which improves the appearance of fine lines, wrinkles, over all skin texture and pigmentation.  You can start to see results with just one treatment but we recommend 3-4 treatments, spaced 4-6 weeks apart, for maximum results. Once you finish the series of treatments you can maintain the results with one treatment per year.  

Myself, and many of my patients, have extremely busy lifestyles and can’t afford to take time off from work and life to have ablative laser treatments that require 5-7 days of aftercare.  With the ResurFX Laser you can expect to be back to your regular schedule the very same day. Most people have a “mild sunburn” appearance for 1-2 days following treatment. There can be some mild swelling as well but nothing major.  We recommend staying out of the sun for a few days post treatment and, as always, wear your sunscreen!

The order in which you receive Aesthetic treatments is important

Botox before facials? Facials before lasers? In the world of aesthetics there are many types of treatments that range from noninvasive facials, aesthetic injections to comprehensive skin resurfacing lasers.  It’s important to receive these in the proper order to ensure the best aesthetic outcome.

Let’s start off with facials.  It is always best to receive your facial before any aesthetic injections. A facial involves massaging and rubbing of the face which can move the product to an area it was not intended for. Vigorous skin treatments such as microdermabrasion and microneedling could also cause the product to spread. If a neurotoxin, like Botox, disperses to a nearby muscle a side effect is droopy skin. This is commonly seen in the eyelid/brow area.  Therefore as a precautionary measure it is best to get the facial/vigorous treatment first or wait at least 2 weeks after receiving injections.

Lasers and treatments that are heat based are ideal before aesthetic injections. Most dermal fillers are made up of hyaluronic gel, which attract water and expand. Lasers that use heat may diminish the water content in the skin and the product, depending on the depth of the injection. Neurotoxin like Botox and Dysport are used to address wrinkles associated with facial animation. Heat causing treatments may cause swelling which could possibly diffuse the product to other areas. This means it is best to receive the laser treatment before any aesthetic injections.

On the outside someone may appear to be calm and relaxed however, an interior view of their mouth tells a very different story.  One of the exciting things about medicine is often we discover that a particular drug can offer benefits to several other aliments then its intended purpose. I suffer from unconscious clenching and grinding of my teeth both during the day and as I sleep. While attending a training workshop I heard Botulinum toxins found in products such as Botox® and Dysport® can be used to relax and relieve the masseter muscle.

After further researching bruxism (the formal name for this condition) I discovered I was not alone and this is a common and painful problem that can lead to severe dental issues. Chronic teeth clenching/grinding can cause headache, earaches, facial pain, and even migraines. Bruxism sufferers who grind and clench their teeth while sleeping frequently wake up with a sore jaw.  The good news is treatments of Botox/Dysport injected into the masseter muscle can provide tremendous relief from jaw soreness, headaches, and other unpleasant problems.  The administration technique is the same which is used for wrinkle reduction. Volbella® Filler

I often times have patients come in bothered by the vertical lines around their mouth or patients who just want a subtle volume enhancement to the lips.  There is now a product made just for these issues, Volbella® by Allergan. Volbella is temporary filler made of Hyaluronic Acid. The uniqueness is that it has a smaller particle size then Juvederm allowing us to fill more fine superficial lines without getting a “filled” look.  This also makes it a great filler for someone wanting just a slight enhancement to the lips and still maintain a natural appearance.

 

Volbella is made using Allergan’s patented Vycross technology.  This technology makes for a smooth gel consistency and should last approximately a year. The side effects are similar to using any other filler and include, swelling, bruising, tenderness and redness at the injection site. These side effects are temporary and vary by patient.  It is important to work with a highly trained medical professional for safety and to ensure you achieve the desired results.

Let’s talk about Fillers!

“Fill me up Doc!” Let’s talk about fillers.  Often times when patients come in to see me they know they are seeing volume loss in their faces, but are not quite sure where or how to fix it.  I am asked on a daily basis how I decide which filler to use. While there are several types of fillers out there, for this blog I’m going to discuss the Restylane group of derma fillers by Galderma.

 

 

There are five different Restylane fillers; Restylane-L, Restylane Lyft, Restylane Silk, Restylane Refyne, and Restylane Defyne.  All these fillers are made of Hyaluronic Acid (HA). All of the HA fillers products are tested for firmness and flexibility.

Restylane and Restylane Lyft are all designed for natural looking results with NASHA (Non-Animal Stabilized HA) Technology.   This technology creates a firmer gel texture for a more pronounced lifted result. Restylane is typically used for facial wrinkles and folds, tear troughs and lips.

Restylane Lyft is most often used in cheek augmentation and mid-face contouring deficiencies.  It is also the first and only HA approved to treat hands. Restylane Silk was produced to work for subtle lip enlargement and for the lines around the lips.

Restylane Refyne and Restylane Defyne were created for movement and expression.  These were designed with XpresHAn Technology, which means they are formulated to act like your body’s own naturally occurring HA.  They give just the right amount of lift and have the right amount of flexibility to allow you to show a range of facial expressions without looking unnatural.  The only difference between to two products is the amount of crosslinking of the HA. Restylane Refyne I like to use for fine lines anywhere on the face or for subtle lip enhancement. Restylane Defyne   I use this for areas on the face where more volume is needed to be restored and for deeper lines of expression.

As you can see from the video the consistency of the products in the Restylane family is quite different which is due to the science behind the development. Does the loss of volume in your face have you feeling down or do you keep hearing people tell you that you have a frown?  I see patients all the time complaining that they feel like their faces are falling or that they are getting “jowly,” or look like they are constantly frowning. These complaints are usually due to us subtly or not so subtly noticing signs of aging on our face.   These signs of aging usually start with changes in the skin, fat and muscle layers as well as changes to the bone structure itself.

The perceived volume loss occurs due to fat redistribution, and the bones become thinner and flatter in places.   As we age, this fat/volume loss in the face causes the face to appear “saggy.” Many patients come in wanting to fill in the nasolabial folds and marionette lines, when often times, restoring volume to the cheek area would be much more beneficial.  Injecting filler into the lower face may not always be a good idea, depending on the patient, because it may possibly accentuate the heaviness a patient is feeling. As we age our faces lose their shape. This causes your heart, oval, or round shape to become more square and bottom heavy.  This facial shape of aging can many times be reversed to some degree by restoring volume to the mid-face. This in turn will impact the appearance of the lower face.

Stop Sweating and Raise Your Arms with Confidence!

Do you dread summer because you can’t raise your arms in the air without showing
wet perspiration stains? Have you tried every deodorant on the store shelves and
nothing seems to control your perspiring? If you answered yes, then I have a
suggestion for you and it’

s Botox®. I’m sure many of you are thinking Botox is for
the wrinkles on my face not my sweaty underarms. It is used for both areas. Studies
have shown that Botox treats overactive sweat glands in your armpits (medically
known as hyperhidrosis) by blocking the uptake of acetylcholine, one of the
neurotransmitters that stimulates sweat.


When Botox is used in the face it freezes the nearby muscles which smoothes
wrinkles and prevents expression lines that could cause even more wrinkles to
form. It is a similar mechanism that is at work when Botox is used to stop
perspiration. Instead of working on nerves that stimulate the muscles, it blocks the
nerves controlling sweat glands essentially “paralyzing” them and significantly
reducing the amount you sweat under your arms. Injecting Botox into this area will
not affect other areas of the body. Your body can still sweat through other areas to
regulate its internal temperature.

If you like the sound of this your next question is likely, what is this process like?
The entire process is usually scheduled as a 30 minute appointment. We will
perform a simple starch iodine test to see where the sweat glands are to better
target the most active glands. Once the area is identified it is cleaned off and the
Botox is injected in small doses. The pain level is tolerable and similar to Botox
injections into facial areas. Most common side effects are bruising and possibly a
little tenderness.

In a clinical study by Ada Regina Trindade de Almeida, MDa,*, Suelen Montagner, MDb, the average
duration of response in patients receiving a Botox treatment for hyperhidrosis was
6 months.
