Volbella® Filler

I often times have patients come in bothered by the vertical lines around their mouth or patients who just want a subtle volume enhancement to the lips.  There is now a product made just for these issues, Volbella® by Allergan. Volbella is temporary filler made of Hyaluronic Acid. The uniqueness is that it has a smaller particle size then Juvederm allowing us to fill more fine superficial lines without getting a “filled” look.  This also makes it a great filler for someone wanting just a slight enhancement to the lips and still maintain a natural appearance.

 

Volbella is made using Allergan’s patented Vycross technology.  This technology makes for a smooth gel consistency and should last approximately a year. The side effects are similar to using any other filler and include, swelling, bruising, tenderness and redness at the injection site. These side effects are temporary and vary by patient.  It is important to work with a highly trained medical professional for safety and to ensure you achieve the desired results.

Let’s talk about Fillers!

“Fill me up Doc!” Let’s talk about fillers.  Often times when patients come in to see me they know they are seeing volume loss in their faces, but are not quite sure where or how to fix it.  I am asked on a daily basis how I decide which filler to use. While there are several types of fillers out there, for this blog I’m going to discuss the Restylane group of derma fillers by Galderma.

 

 

There are five different Restylane fillers; Restylane-L, Restylane Lyft, Restylane Silk, Restylane Refyne, and Restylane Defyne.  All these fillers are made of Hyaluronic Acid (HA). All of the HA fillers products are tested for firmness and flexibility.

Restylane and Restylane Lyft are all designed for natural looking results with NASHA (Non-Animal Stabilized HA) Technology.   This technology creates a firmer gel texture for a more pronounced lifted result. Restylane is typically used for facial wrinkles and folds, tear troughs and lips.

Restylane Lyft is most often used in cheek augmentation and mid-face contouring deficiencies.  It is also the first and only HA approved to treat hands. Restylane Silk was produced to work for subtle lip enlargement and for the lines around the lips.

Restylane Refyne and Restylane Defyne were created for movement and expression.  These were designed with XpresHAn Technology, which means they are formulated to act like your body’s own naturally occurring HA.  They give just the right amount of lift and have the right amount of flexibility to allow you to show a range of facial expressions without looking unnatural.  The only difference between to two products is the amount of crosslinking of the HA. Restylane Refyne I like to use for fine lines anywhere on the face or for subtle lip enhancement. Restylane Defyne   I use this for areas on the face where more volume is needed to be restored and for deeper lines of expression.

As you can see from the video the consistency of the products in the Restylane family is quite different which is due to the science behind the development. A good injector must understand all the products ingredients, where and how they are to be administered and what you the patient’s desired outcome is.  Be sure to choose a qualified licensed medical professional for your treatment.

Add Volume to Your Face Without Surgery

My face is starting to sag….What can I do without surgery?

Does the loss of volume in your face have you feeling down or do you keep hearing people tell you that you have a frown?  I see patients all the time complaining that they feel like their faces are falling or that they are getting “jowly,” or look like they are constantly frowning. These complaints are usually due to us subtly or not so subtly noticing signs of aging on our face.   These signs of aging usually start with changes in the skin, fat and muscle layers as well as changes to the bone structure itself.

The perceived volume loss occurs due to fat redistribution, and the bones become thinner and flatter in places.   As we age, this fat/volume loss in the face causes the face to appear “saggy.” Many patients come in wanting to fill in the nasolabial folds and marionette lines, when often times, restoring volume to the cheek area would be much more beneficial.  Injecting filler into the lower face may not always be a good idea, depending on the patient, because it may possibly accentuate the heaviness a patient is feeling. As we age our faces lose their shape. This causes your heart, oval, or round shape to become more square and bottom heavy.  This facial shape of aging can many times be reversed to some degree by restoring volume to the mid-face. This in turn will impact the appearance of the lower face.

Derma filler such as Voluma ® or Restylane Lyft ® to the mid-face will also likely help lift the nasolabial folds and the marionette lines in the lower face as well as at times minimize or improve the appearance of jowls. Fillers can also make the skin appear smoother, plumper and help minimize fine lines in certain areas. The lips are one of the most important features of the face. When we are talking to someone, we not only notice their lips, but unconsciously or subconsciously evaluate them. Am I Too Young for Botox?

 

Some would say Botox®/Dysport® before wrinkles is useless but I beg to differ.  As someone who didn’t start Botox til my late 30’s, I certainly wish I could turn back time!  Botox or Dysport can and is much more frequently being used as preventative maintenance for wrinkles. If you smile a lot (crows feet) or frown a lot (Glabellar or 11’s) these products in your 20’s can prevent the wrinkles from setting in!

By watching someone’s facial expressions you can see where they are most likely to form wrinkles in the future.  Those are the areas where you would want to start these products early. It actually trains the muscle to relax. The good news is you can usually use a smaller dose when using Botox/Dysport preventatively.  You also do not need to be completely frozen for it to work! You can still have some expression while preventing deep lines from setting in! I think it’s a win win!!

Do you have dark circles under your eyes? Are people constantly asking you if you’re tired?

Dark circles or bags under your eyes may be present from birth or can develop with age.  They make us look older and appear tired. As an Aesthetic RN at Dr. Schenden’s in Troy, Michigan, I use derma fillers in the “tear trough” area to minimize the hollowness and often dark area. This treatment softens and contours the area making you look less tired.

What is a tear trough?

Tear trough or under the eye hollows are well defined dark and deep hollows that are located between the lower eyelid, nose and upper cheek.

Injection Technique

My preference is to use a blunt tip micro cannula. The skin in this area is very thin and delicate. Prior to treatment I inject the area to numb the injection site. I then use a needle to create an insertion point below your tear trough. Using this technique allows me to use 1 injection site utilizing the cannula to place the product with more precision and less trauma to the area. The use of the cannula is a safer practice. My preferred micro cannula is a round tipped blunt needle that will simply push tissue, blood vessels and nerves out of the way.

Does it hurt?

My patients experience less discomfort utilizing this technique. In addition it reduces the risk of swelling and bruising. The derma filler injected contains lidocaine to help numb the area as well the initial injection.

What are the Side Effects?

With any injection there is the possibility of bruising, redness and swelling. Discontinuing Aspirin, Motrin, Ibuprofen, NSAID’s, Fish Oil, CLA and other blood thinning products 1 week prior to your appointment will decrease the possibility of bruising.

How long do results last?

Hyaluronic fillers last anywhere from 9-12 months.  The under eye area is relatively static and doesn’t animate as much as other areas, creating the potential to last longer.
